    Transition Services

    Section 5 of the IEP is required for any student turning fourteen during the course of the IEP, or any student who is fourteen years of age or older.

    All students in grades 7-12 who are currently placed in a low incidence or medically fragile classroom will automatically be considered as part of the Transition Coordinator's (TC) caseload, which has a maximum of 75 students. Your TC will be able to administer Age-Appropriate Transition Assessments (AATA) and other services to these students, as well as others on their caseloads who are not in low incidence settings, or not coded MH/MD or Autistic. The TCs are also available to share transition resources with Intervention Specialists servicing students not on the TC caseloads. TCs do not input information into the IEPs.
